Office Hours and Scheduling
Our office is open Monday through Friday from 8:00 a.m. to 5:00 p.m. The office is closed on weekends and all major holidays. Patients or referring physicians may schedule appointments by calling: (308) 865-2540. The time allotted for each appointment depends on the nature of the problem. If you must change, or cannot keep your appointment, please advise the office as soon as possible by calling: (308) 865-2540. If you are visiting us for the first time, please arrive 15 minutes before your scheduled appointment to allow time for registration. Please bring your medical insurance card(s) and a list of ALL medications you are currently taking. This includes prescription,
herbal, and over the counter medications.

Referrals
Many of the patients seen in our office are referred to us by their family physicians. If you have been referred to us by your family physician, we will keep them advised as to your treatment and results. If you have not been referred, we will be happy to keep your personal physician informed, if you wish.

Telephone Procedure
If you have a medical question, please call (308) 865-2263 during office hours which are 8:00 a.m. to 4:30 p.m. Monday through Friday. Our nurses can assist you with most questions, arrangements will be made to have the doctor return your call.

Urgent/Emergency Calls During Office Hours
If possible, we urge that you phone (308) 865-2263 before coming to the office to ensure
immediate attention upon your arrival. Your doctor will see you if they are available, otherwise another physician will provide treatment.

Emergency Call At Times Other Than Regular Office Hours
You may reach the physician on-call after normal working hours by calling our office number (308) 865-2263. The call will be answered by our answering service. The answering service will locate the physician on-call and have them call you back in a short time. If your personal physician is not on-call, the physician on-call will care for you until your personal physician returns. If you cannot wait for the physician to return your call, advise the answering service that you are going to the Emergency Room. Emergency Room visits are initially handled by the Emergency Room physician at Good Samaritan Hospital. If necessary, your Platte Valley Medical Group physician on-call will see you.

Finance Charges & Refunds
A finance charge will be assessed on all charges that are over 90 days old. Our monthly finance charge is 1.0%. Our annual finance charge is 12%.

Patient Confidentiality
The information in your medical history file is confidential and will be released to others only with your written consent.

Insurance
Please present your insurance identification card and any required
claim forms at the time of your visit. Our office will submit your insurance claims, including Blue Cross and Blue Shield of Nebraska, Medicare claims, and Medicaid. WE will need your policy numbers and the address of your insurance company. After your insurance carrier has paid their portion of the bill you will receive an explanation of benefit statement indicating your final payment information.

If you have Medicare supplement insurance, please present this insurance card as well as your Medicare card. We will bill your supplement insurance for you. The Medicare payment will be sent directly to Platte Valley Medical Group. The Medicare supplement payment may be sent to us or to you. If you do receive this payment please forward to Platte Valley Medical Group as soon as possible.

Worker's Compensation
If you are eligible for Worker’s Compensation benefits, please tell the receptionist at the front desk so we do not bill this to your private insurance. The insurance form will be completed by our insurance department and mailed to you to take to your employer.

Disability Insurance
If you are eligible for disability benefits, the disability form will be completed by our insurance department. The disability form must be completed and signed by the disabled party when you bring it to us for completion.

Non-Emergency
Non-Emergency calls are usually referred to your doctor’s nurse. If the nurse is busy with other patients, you may be asked to leave a message with our telephone operator and your problem will be handled as soon as possible.

Walk-In Patients Welcome
Individuals requiring health care for minor illnesses such as, sore throats, sinus issues, ear aches, stomach pain, and so forth may call or walk-in and one of our General Internists will be available to address your health care needs.

To Better Serve You

We would ask that you bring all your medications to the appointment.
Doing so will assist us in reducing your waiting time. Patients needing injections, lab, and/or blood pressure checks are encouraged to call our office in advance.

Patient Fees

Payment for all charges is required at the time the service is rendered, unless prior payment arrangements have been made with your insurance carrier. Patient fees vary according to the type and extent of service provided. We strive to keep our fees reasonable by reviewing our operational costs annually. Statements are processed monthly. If, for some reason, you are having a personal financial crisis, you should contact our Collections Department Representative to discuss a payment schedule. That number is (308) 865-2808.
